In an effort to reduce the impact of polypharmacy for the hospital’s physical therapy patients – as well as inpatients, Carthage Area Hospital now offers a new service to assist in addressing these issues. This is especially important in the elder adult with polypharmacy contributing to elevated risk of falls.
Referral and Medication Management Consultation Criteria (with Clinical Pharmacist, Christopher Bradley):
1) 3 or more of the following chronic disease states:
- Diabetes
- Chronic Heart Failure
- Dyslipidemia
- Hypertension
- COPD
- Osteoporosis
- Asthma
- Depression
- Rheumatoid Arthritis
- End-Stage Renal Disease
- HIV/AIDS
- Other Cardiovascular Disorders
- Osteoarthritis
- Alzheimer’s/Dementia
2) Taking 8 or more prescription medications
3) Out of pocket annual copay expense of at least $3500 at the pharmacy
